HELP FOR YOUR PET

If you are experiencing a medical emergency with your beloved pet and are also experiencing financial hardship, all that is required from the pet owners seeking help is a complete application to determine eligibility.

A complete application is necessary to be submitted for review prior to receiving assistance. Please read through ALL the information below on what consists of a complete application, including FACE’s Terms and Conditions, and see if you are eligible for a grant from FACE. We look forward to helping you and your pet.

WHO QUALIFIES FOR FACE FUNDING?

  • FACE provides financial grants for pets in need of emergency or critical veterinary care.
  • Pets must have already been seen by a veterinarian within the past two weeks and your veterinarian has determined that your pet has an immediately life-threatening or critical care condition.
  • Pets must have a good prognosis with treatment.
  • FACE can only provide assistance to pet owners residing in San Diego County.

WHO DOES NOT QUALIFY FOR FACE FUNDING?

Please note that FACE cannot provide assistance for the following:

  • Initial exam/consultation fees
  • Routine or non-emergency care (i.e. Vaccines, Spay/Neuter, Cruciate ligament tears, etc.)
  • Services that have already taken place and/or treatment completed prior to FACE approval
  • Conditions requiring ongoing treatment (i.e. Diabetes, Addison’s disease, Heart disease, etc.)
  • Conditions with a poor or guarded prognosis (i.e. kidney failure, etc.)
  • Please note FACE can only assist pet owners up to two times.

How Do I Apply?

ALL of the following must be submitted by all adults in the household:

IF YOU ARE APPLYING AFTER 5PM OR ON THE WEEKEND, FOLLOW THE AFTERHOURS INSTRUCTIONS BELOW. INCOMPLETE APPLICATIONS WILL NOT BE REVIEWED.

  1. Care Credit and Scratch Pay results (Scratch Pay if applicable). (screenshot photos for each)
  2. Screenshot of Current Bank balances in ALL bank accounts and lines of credit (checking, savings and credit accounts). Examples of Accounts include- Chime, EDD Bank of America, Direct Express, Venmo, personal bank accounts, ALL credit card companies.
  3. Complete 30 days of current bank transactions for these accounts(screenshot photos/pdf statement will suffice)
  4. Photos of your pet including Holiday themed photos.
  5. Also include your link to fundraising campaign–(start your campaign here: https://www.gofundme.com).
  6. FACE Online Financial Assistance Application (RFA Form).

Please submit all items required and attach to your online application. Read the “How to submit my documents” for more information on submitting the required items.

*Please note any approved credit will need to be utilized towards any tests for diagnosing your pet prior to applying for FACE, and/or utilized towards your pet’s procedure. FACE cannot always provide funding for the entire cost of the procedure, and recommends reaching out to other organizations, family, and friends to secure additional donations for your pet as well. *

How to submit your documents

Documents can be attached to your online application as screenshot photos and PDF’s and emailed to [email protected], as well as faxed to 858-457-3223. If these options are not available to you, please speak with your veterinary hospital on helping you submit your documents.

Medical portion REQUIRED for review:

  1. FACE RFA Form signed by the treating veterinarian. “Hospital Use Only” Box.
  2. Treatment estimate from the treating veterinary hospital
  3. Complete Medical records for the current diagnosis.

AFTER HOURS: If you are applying after 5PM or on weekends FACE is unable to receive any phone calls as we are no longer available to receive phone calls. All paperwork must be emailed or faxed. Once your complete application is received, please CHECK YOUR EMAIL. FACE will notify both the pet owner and treating veterinarian of approval or denial once a complete application is received.

Notes on Receiving FACE Funding

Terms and Conditions:

  • All animals that receive FACE funding must be spayed or neutered. If an animal is not already spayed/neutered, it will be spayed/neutered at the time of the procedure. If this cannot be performed at the time of treatment, the owner will agree to have their animal spay/neutered within a set time frame (based on the animal’s health). This will be performed at the owner’s expense and is not covered by the initial grant.
  • Orthopedic surgeries must be done by a board-certified surgeon.
  • Funds are paid directly to the treating veterinary hospital/clinic.
  • Veterinarians who partner with FACE must provide a 25% discount for FACE cases.
  • Recipients of FACE funding agree to allow FACE to use their story, photos, and video.
  • Recipients are asked to do whatever possible to help FACE save other family pets. This includes signing up for a monthly donation paying it forward, volunteering, and participating in special events.
  • FACE strongly encourages all grantees to consider pet insurance such as Embrace or Healthy Paws.

Cómo Se Inscribe

Todo lo siguiente debe ser enviado:

  1. Forma de Solicitud de Asistencia Financiera.
  2. Dueños de mascotas que viven en el Condado de San Diego se pueden usar nuestra aplicación en línea (si se inscribe en línea, asegúrese de leer el siguiente email enviado por FACE) o descarga la FORMA RFA (clic aquí para el versión en Español)
  3. El hospital que les trata debe llenar la parte encajada del RFA, bajo de la firma del dueño.
  4. Una estimación de tratamiento del hospital veterinario.
  5. Archivos médicos completos, incluidos radiografías, cuando necesario, del hospital veterinario tratante.
  6. Resultados de la solicitud “Care Credit” de todos los adultos en el hogar (solicítelo en www.carecredit.com o en su hospital veterinario).
  7. Fotos y videos de la mascota.
  8. Prueba de dificultades financieras por todos en el hogar a través:
    • Documentación de los totales actuales y disponibles en sus cuentas de cheques y los ahorros (si corresponde).
    • Un mes de sus transacciones actuales del banco (si pertinente).
    • Si los dueños de mascotas no tienen cuentas bancarias, otras formas aceptables de ingresos incluyen:
      • Formulario del impuestos 1040
      • Ingresos del Seguro Social (si corresponde)
      • Papeleo de desempleo o asistencia del gobierno

Cuando se reciba su solicitud complete, revise su email. FACE les notificará al dueño de mascota y al veterinario tratante sobre la aprobación o denegación cuando se haya recibido la solicitud completa. Por favor, tenga en cuenta que FACE no siempre puede proporcionar fondos para todo el costo del procedimiento, por eso le recomendamos que se contacta con otras organizaciones, familiares, y sus amigos para obtener más donaciones para su mascota. Términos y Condiciones

  • Todos los animals que reciban fondos de FACE tienen que ser esterilizado o castrado. Si un animal ya no es esterilizado o castrado, sera hecho a la hora del procedimiento. Si esto no puede ser realizado a la hora del tratamiento, el dueño se debe acordar tener el animal esterilizado/castrado dentro de un marco de tiempo específico (segun el salud del animal. Sera hecho a expensas del dueño y no sera pagado por la beca inicial.
  • Las cirugías ortopédicas deben ser realizadas por un cirujano junta-certificado.
  • Los fondos son pagados directamente al hospital veterinario.
  • Los veterinaries que asocian con FACE deben proporcionar un descuento del 25% para los casos de FACE.
  • Los destinatarios de los fondos de FACE acuerdan permitir que FACE use su historia, fotos y video.
  • A los destinatarios se les pide que hagan lo posible para ayudar a FACE a salvar a otras mascotas de la familia. Esto incluye estar voluntario y participar en eventos especiales.
  • FACE recomienda fuertemente a todos los beneficiarios considerar el seguro de mascotas.
